Overview

Why critical care billing is high-risk and high-value

Critical care services are among the highest-value E/M services, but they come with strict requirements. The patient must have a critical illness or injury with a high probability of imminent deterioration, and the physician must document total critical care time, excluding separately billed procedures and time spent on non-critical activities.

Intensivists and hospitalists also perform procedures such as intubation, central lines and arterial lines that may be billed separately, while other services like ventilator management and blood gas interpretation are bundled into critical care. Knowing the difference is essential to avoid both under-billing and overpayment risk.

Because ICU coverage involves multiple physicians, nurse practitioners and physician assistants, billing must follow rules for split/shared visits and prevent duplicate claims. A critical care billing partner reconciles hospital census data against submitted charges every day so no encounter is lost.

Why it's hard

Common critical care billing challenges

01

Time documentation

Total critical care time must be documented and must exclude time spent on separately billed procedures.

02

Bundled vs. separate procedures

Some services are included in 99291; others like intubation or central lines can be billed separately.

03

Split/shared and team visits

Multiple clinicians in one day require careful attribution to avoid denials and duplicate billing.

04

Medical necessity scrutiny

Payers review whether the patient truly met critical care criteria at the time billed.

05

Missed charges from the ICU

Rounding notes, consults and night coverage often never reach billing without daily reconciliation.

06

Transitions between levels of care

Patients moving between ED, ICU and floor need the right code family each day.

Key codes in critical care billing

Code(s)What it covers
99291Critical care, first 30–74 minutes of total time on a given date.
99292Each additional 30 minutes of critical care (add-on to 99291).
99221–99223Initial hospital inpatient or observation care.
99231–99233Subsequent hospital inpatient or observation care.
31500Emergency endotracheal intubation.
36556 / 36620Central venous catheter insertion / arterial line placement.
99468–99476Neonatal and pediatric critical care, initial and subsequent days.

Code examples are for orientation only. Always code from the current CPT®, HCPCS and ICD-10-CM code sets and payer policies.

Documentation that holds up

Every critical care note should state the critical condition, the interventions provided, and total time spent, excluding procedure time. Billing partners should audit a sample of notes regularly and give providers feedback before payers do.
