Overview

How OB/GYN billing works

Routine obstetric care is usually billed as a global package that covers antepartum visits, delivery and postpartum care. The package is billed once after delivery, so practices must track each pregnancy for months and make sure the right global code is used for vaginal delivery, cesarean or VBAC.

Not everything is included in the global. Ultrasounds, non-stress tests, treatment of complications, and visits for unrelated problems can be billed separately with the right diagnosis codes. When a patient changes practices or insurance mid-pregnancy, the global must be broken into separate antepartum, delivery and postpartum codes.

Gynecology adds preventive well-woman visits, contraception, colposcopy, hysteroscopy and laparoscopic surgery. An OB/GYN billing partner tracks every pregnancy from first visit to postpartum, captures services outside the global, and handles insurance changes and Medicaid rules along the way.

Why it's hard

Common ob/gyn billing challenges

01

Global package tracking

Pregnancies span months; without tracking, globals are billed late or not at all.

02

Split and transferred care

Patients who switch practices or insurers need the global broken into components.

03

Services outside the global

Ultrasounds, NSTs and complication visits are missed when bundled by mistake.

04

High-risk pregnancy documentation

Extra visits for high-risk conditions need diagnoses that justify separate billing.

05

Preventive vs. problem visits

Well-woman visits with problem care need correct coding and modifier 25.

06

Medicaid and eligibility changes

Pregnancy Medicaid and coverage changes require frequent eligibility checks.
