Why podiatry billing trips up practices
Medicare generally excludes routine foot care such as nail trimming and callus paring, unless the patient has a qualifying systemic condition and documented class findings. Claims then need the right Q7, Q8 or Q9 modifier, the treating physician’s information and dates, and covered diagnoses.
Podiatric procedures also require precise anatomic modifiers. Every toe has its own modifier, and missing them leads to duplicate-claim denials when multiple toes are treated. Wound care, debridement and foot and ankle surgery bring additional coding rules and global periods.
Many podiatry practices supply diabetic shoes and inserts, orthotics and other DME, which have their own certification, documentation and supplier enrollment requirements. A podiatry billing partner handles all of these rules so practices are paid for every service they provide.
Common podiatry billing challenges
Routine foot care coverage
Covered only with qualifying conditions and class findings documented and coded.
Q7, Q8 and Q9 modifiers
Class findings modifiers must match the documented exam findings.
Toe modifiers
TA and T1–T9 modifiers are needed to distinguish procedures on different toes.
Diabetic shoe requirements
Therapeutic shoes need certifying physician statements and specific documentation.
Wound care and debridement
Depth and surface area determine codes; documentation must be precise.
Surgical global periods
Bunion and hammertoe surgeries have 90-day globals requiring modifiers for unrelated care.

Routine foot care documentation
For Medicare routine foot care, document the qualifying systemic condition, class findings, and, when required, the name of the physician actively managing the condition and date last seen. Missing these is the top podiatry denial reason.
