Overview

What is revenue cycle management?

Revenue cycle management (RCM) is the process a healthcare practice uses to turn patient visits into collected revenue. It starts before the patient arrives, with insurance verification, prior authorization and cost estimates, continues through charge capture, coding and claim submission, and ends only when the payer and patient balances are fully paid or appropriately adjusted.

Most revenue is lost in the gaps between those steps: an eligibility check that was skipped, a missing modifier, a denial nobody appealed, or a patient balance that never received a statement. Industry studies consistently show that a large share of denied claims are never reworked, which means money the practice already earned is simply written off.

An RCM partner owns the whole cycle and measures it. Instead of reacting to problems, a good RCM team tracks clean-claim rate, first-pass resolution, denial rate, days in A/R and net collection rate, finds the root cause of every denial category and fixes it upstream, at registration, in documentation or in the coding rules.

On BillerFirms you can compare RCM companies that work with practices like yours, read verified client reviews and request a free audit of your current revenue cycle.

Why it's hard

Common billing challenges

01

Front-end registration errors

Wrong subscriber IDs, outdated coverage and missing referrals cause a large share of denials before a claim is even coded.

02

Rising denial rates

Payers continually add edits, prior-authorization rules and documentation requirements, pushing denial rates up year after year.

03

Unworked A/R

Small balances and older claims are easy to ignore. Without disciplined follow-up they age past timely-filing limits and are lost.

04

Staff turnover and training

Billing staff are hard to hire and retain. Each departure means lost payer knowledge and a backlog that keeps growing.

05

Patient responsibility

High-deductible plans shift more of each bill to patients. Without estimates and clear statements, patient collections fall behind.

06

No visibility into performance

Many practices cannot see their real denial rate, payer mix or collection rate, so they cannot tell what is going wrong.

Benchmarks

RCM metrics every practice should track

MetricWhat good looks like
Clean-claim rateShare of claims accepted on first submission. Aim for 95% or higher.
Denial rateShare of claims denied by payers. Best-performing practices keep it under 5%.
Days in A/RAverage time to collect. Under 35 days is strong; over 50 signals problems.
A/R over 90 daysPortion of receivables older than 90 days. Keep it below 15% of total A/R.
Net collection rateCollected vs. collectible after contractual adjustments. Target 96% or more.
Charge lagDays from date of service to claim submission. Same-day to 3 days is ideal.

Benchmarks vary by specialty and payer mix; use them as a starting point when you compare RCM companies.

Ask for proof, not promises

Before you sign, ask each RCM company for de-identified KPI reports from current clients, their denial-management process, who owns your data and how it is returned if you leave, and a signed HIPAA Business Associate Agreement.
