Job Overview

MedCare MSO Global is looking for a motivated and detail-oriented Patient Enrollment Specialist to join its team. The ideal candidate will play a key role in managing patient enrollments for the Chronic Care Management (CCM) program while ensuring a smooth and professional patient experience.

Key Responsibilities

• Manage the end-to-end CCM patient enrollment process
• Educate patients about CCM program benefits and eligibility criteria
• Coordinate with healthcare providers and Nurse Practitioners (NPs) to verify patient eligibility and required documentation
• Track and monitor patient enrollment progress and ensure timely follow-ups
• Work closely with the Sales Team to achieve enrollment goals and improve patient outreach
• Maintain accurate patient enrollment records and documentation
• Ensure compliance with applicable CCM program requirements and procedures

Requirements

• Bachelor’s degree is mandatory
• 1–2 years of experience in healthcare enrollment, patient services, or a related role
• Strong understanding of Chronic Care Management (CCM) programs and regulatory compliance
• Excellent verbal and written communication skills
• Strong attention to detail and organizational skills
• Proficiency in healthcare management software for patient tracking and communication
• Ability to work effectively in a fast-paced, target-driven environment
